This is a very common type of case in patients with advanced periodontal disease and severe functional overload: they first notice slight tooth mobility, and then, suddenly, that mobility becomes generalized, especially in the upper incisors. That is when the aesthetic problems begin, along with insecurity when smiling and real difficulty eating.

This patient came to my clinic referred by other patients, presenting with a compromised periodontal condition, marked functional overload, and severe mobility of the upper incisors. He was a patient with strong masticatory force, significant bone volume, but poor bone quality.

 

With conventional implantology, this usually means multiple extractions, healing periods, provisional removable prostheses, bone grafting, and, finally, a bulkier fixed rehabilitation, because the prosthetic space is no longer that of natural teeth.

 

Here, we did something different.

 

We extracted the four upper incisors and, during the same surgical procedure, placed four 23 mm basal anchorage implants, seeking support in the cortical bone, the densest and most stable bone available. In addition, intraoral welding was performed to rigidize the entire structure and provide maximum stability from the very first moment.
