FIXED TEETH IN JUST ONE DAY, WITHOUT BONE GRAFTS…
Hello! I’m Dr Darío Castro. If you are reading this, it is likely that at some point you have been told that “you do not have enough bone”, that you need long and painful bone grafts, or that you must wait months before being able to eat normally again.
Today, I want to share a real story with you. The story of a patient who travelled from the neighbouring island of Fuerteventura to our clinic in Las Palmas de Gran Canaria in search of something that seemed impossible: to regain his chewing function and his smile immediately, without endless procedures and with complete confidence.

The problem: When time and mobility are the greatest enemies
Our patient was facing a complicated situation. He had just lost an upper left premolar and had already been missing several lower molars for quite some time. As he lived on another island, his availability to travel was limited. He simply could not commit to a treatment requiring ten visits a year.
At other clinics, the proposed solution was always the same: “We will carry out bone grafts, wait six months for healing, then place the implants and wait another four months…”. In total, almost a year before being able to chew properly again.
My approach, both as a lecturer and clinician, has always been different: why wait when the patient’s anatomy already offers us solutions today?
The solution: Basal Implantology and Immediate Loading
We decided to apply a basal implantology protocol. Unlike traditional implants, which are placed where the tooth used to be (and where bone is often lacking), basal implants are anchored in deep areas of the face that are extremely stable and rich in minerals.
Upper Sector: The power of biomechanics
In the upper left side, where the lack of bone is usually a major issue, we placed a bipod implant. We strategically anchored it towards the posterior nasal floor, an area of very high stability. To reinforce it, we added a Scialom needle.
Sounds complex? Imagine it like the pillars of a bridge: we place them in a divergent (tilted) manner so that, when you bite, the forces are distributed and the implant does not suffer. The result: immediate stability.
Special for patients with Bruxism: Extreme resistance
A vital detail: our patient suffers from severe bruxism (he clenches his teeth heavily). For him, a conventional implant could be risky. Therefore, in the lower right sector we used AZ-type monoblock implants.
These implants are “one solid piece” of titanium. They are incredibly resistant and are designed to withstand brutal chewing forces without fracturing. We achieved a stability exceeding 50 Ncm (a very high torque that allows us to place the teeth on the same day).
The “miracle” of immediate loading: Teeth on the same day
Once the implants were placed, we performed an intraoral blade welding. It is like creating a metal skeleton that joins all the implants together, giving them absolute solidity from minute one.
We fabricated the provisional teeth in high-density acrylic and cemented them during that same session. The patient walked into the clinic with missing teeth and left, a few hours later, being able to smile and eat a transitional soft diet.
